Physical Exams

FAA Physical examinations for the private pilot.  $150.  (BasicMed)
This form is to be used by individuals seeking to operate certain small aircraft in accordance with Chapter 14 of Code of Federal Regulations (14 CFR), Section 61.113(i). This rule (BasicMed) allows pilots to use this checklist, and other requirements, in lieu of holding third-class FAA Airman Medical Certificate. Under BasicMed, an individual may only act as pilot-in-command (PIC) of an aircraft that is authorized to carry not more than 6 occupants, and that has a maximum certificated takeoff weight of not more than 6,000 pounds. The examination section of the checklist may only be completed by a state-licensed physician.
Student physicals. $150

A student-athlete may not be permitted to participate in a practice session or in any athletic event (including rifle and cheer) until there is on file, with the superintendent or principal, a statement, signed by a practicing physician, advanced nurse practitioner, physician’s assistant, doctor of chiropractic or a military medical examiner, and the student’s parents or legal guardians, certifying that he or she has submitted to a history and physical examination within the prior 18 months.
